Another option to report a claim is via e-mail within 24 hours of the incident (or as soon as possible):

E-mail: claims@pieinsurance.com

In the e-mail, include the following information:

  • Name of business
  • Policy Number
  • Reporting party’s contact information (name, phone, e-mail)
  • The name, phone and e-mail address of injured employee
  • Date of incident
  • Location of incident (address and/or city, state)
  • Description of incident

Reporting Requirement: 

  • Report all job-related injuries to the insurance company within 7 days of discovery. 
  • Provide a copy of the injury report to the injured worker (Form DFS-F2-DWC-1). 
  • Report required wage information to the insurance company within 14 days of learning of an injury that will require the employee to miss work for more than 7 days or that results in a permanent impairment.

NOTE:  Penalties for late filing of a claim that was due to the employers failure to timely notify the insurer. If the First Report of Injury (DFS-F2-DWC-1) is filed late with the Division, due to the late reporting of the accident by the employer to the insurance company, the employer may be penalized.
